Privacy notice
This notice explains what personal data this website collects, why, and what you can do about it. It is written to meet the requirements of the Digital Personal Data Protection Act 2023 and the DPDP Rules 2025.

What we collect, and why
This website collects personal data only when you choose to send it, through one of two forms.
From the appointment request form:
- Your name and phone number — so we can call you back to arrange an appointment.
- Your email address, if you give it — as an alternative way to reach you, and to send the dental news email if you asked for it.
- What you tell us in the message field — so we can prepare for your visit.
From the patient registration form, if you choose to complete it online: everything on that form, including your medical history, current medicines, allergies, dental history and habits such as tobacco use. This is health data, and we explain it separately below.
We do not use this data for any other purpose, and we do not sell it or share it for advertising.
Please do not send clinical details through the enquiry form
The enquiry form on the contact page is for arranging appointments. Please do not include medical history, symptoms in detail, or images through it. Bring that to your appointment, or phone the clinic.
The online patient registration form
The patient registration form is different, and is the one place on this site where we deliberately ask for health information: your medical history, the medicines you take, your allergies and your dental history. We ask because that information decides what dental treatment is safe for you.
If you complete it online and submit it, those answers travel over the internet to the clinic's email inbox, using the form service Web3Forms. They are then held in the clinic's email account and entered into your clinical record. We ask for your explicit consent on the form itself before any of that happens.
You do not have to send health information this way. The same form can be downloaded and printed, filled in by hand and brought to your appointment, or completed at the clinic when you arrive. Nothing about your care depends on which you choose.
How long we keep it
Enquiries that do not become appointments are deleted within 24 months. Where you become a patient, your contact details become part of your clinical record and are retained in line with the clinic's records policy.
Who processes it
Form submissions — both the enquiry form and the online patient registration form — are delivered to the clinic's email by Web3Forms, which also holds a copy on its own servers for 30 days before deleting it. The dental news email, if you subscribe, is sent by Brevo. This site is hosted by Cloudflare. These providers process data on our instructions only.
Your rights
Under the DPDP Act you may:
- ask what personal data of yours we hold and how it is being processed;
- ask us to correct or complete inaccurate data;
- ask us to erase your data, where we are not required to keep it;
- withdraw your consent at any time, as easily as you gave it;
- nominate another person to exercise these rights on your behalf;
- raise a grievance with us, and escalate to the Data Protection Board of India if it is not resolved.
Withdrawing consent
Email privacy@srishankaradental.comand we will stop processing your data and delete it, unless we are required by law to retain it. To stop the news email, use the unsubscribe link at the foot of any issue.

We aim to respond to any grievance within 30 days.
Cookies and analytics
This site sets no advertising or tracking cookies, and does not use third-party analytics. The only data that leaves your browser is what you choose to type into one of the forms and submit.
The one exception is the map on the contact page. It is supplied by Google, so when that page loads the map, Google receives your IP address and may set cookies in your browser, exactly as on any page that shows one of their maps. We use it because being able to see where the clinic is matters more, for most people, than avoiding that. Nothing else on this site loads anything from a third party.
While you are filling in the online registration form, a draft is kept in your own browser's local storage so you can stop and come back to it. That draft stays on your device, is not sent anywhere, and is erased as soon as the form is submitted. You can clear it yourself at any time with the "Clear the form" button.
Changes to this notice
If this notice changes materially we will update the date below and, where required, contact you.
